Job Summary
We are seeking a dedicated Behavioral Health Professional to join our team. The ideal candidate will provide counseling and therapeutic care coordination services to individuals experiencing behavioral health challenges. Patients may be self-aware or unaware of their challenge. This role requires a compassionate individual with a strong understanding of behavioral health practices and techniques.
Duties
Conduct intake form and monthly assessments to identify clients' behavioral health needs.
Develop and implement treatment plans tailored to each client's unique situation.
Provide brief monthly individual tele care coordination counseling sessions.
Educate patient and caregiver about care goals and importance of well-being.
Collaborate with clinic and care team to ensure patient needs and concerns are addressed.
Maintain accurate and up-to-date patient notes and records
Connect with patients virtually using SMS and emails as appropriate
Qualifications
Bachelor's or Master's degree in Psychology, Counseling, Social Work, or related field.
State licensure or certification as a Behavioral Health Professional
Experience in providing counseling services to individuals with behavioral health disorders
Strong communication and interpersonal skills-
Ability to work effectively with care team and colleagues being remote.
Compassionate, persistent and empathetic approach towards patients.
Provide consistent care and make every effort to support patients as clinically appropriate
Must be timely and professional.
Notify supervisor if taking time off and report to work unless approved for time off
Job Type: Part-time, Choose your own hours: Day shift: Monday to Friday, Weekends if needed, Schedule: 4 hour shift
Pay: $18.00 - $21.00 per hour
