Role Overview:
Under the Medical Director's guidance, the Psychiatric Mental Health Nurse Practitioner collaborates with PACE Physicians and the IDT team to deliver assessment, planning, and education for participants
Key Responsibilities:
Provide psychiatric consultation and treatment for participants, working with PACE and Community-Based PACE Physicians
Conduct in-person or telehealth psychiatric assessments, collaborating with the IDT to create comprehensive care plans
Assist in discharge planning from acute or long-term care settings
Refer participants to psychiatrists as necessary, in collaboration with physicians
Oversee nursing home patient care with regular visits as needed
Act as a consultant to the IDT, updating on participant changes and coordinating care
Job Requirements:
Master's degree in nursing with a Psychiatric/Mental Health emphasis from an accredited institution
Certification as a Psychiatric Mental Health Nurse Practitioner
Active State License and restrictions-free to practice as a PMHNP, along with State CDS certification and a DEA License
NPI license

Salary:
Base salary between $155,000 - $185,000, with bonus potential and equity
