Job Description

The primary focus of the Psychiatric Nurse Practitioner is to provide home-based medical care to chronically ill patients.

Details:

Full-time, permanent
Perform an initial comprehensive assessment on all newly enrolled members
Develop a member management care plan upon enrollment
Establish and maintain effective relationships with members and their legal representatives, staff, PCP’s, and other collaborating mental health providers
Escalate and communicate any issues with severely mentally ill, complex mental health patients, and any active inpatient psych admission
Understand and comply with long-term care policies and procedures
Actively participate in weekly multi-disciplinary team meetings

Qualifications

Current, unrestricted California NP license
Completion of RN accredited program
Completion of accredited program in Psychiatric/Mental Health Nurse Practitioner
Minimum two years clinical experience
Knowledge of CPT, ICD-9, and HCPC codes
