Case Manager for Mobile Crisis Team

Facility: Faith-based, not for profit mental health agency located in Chicago’s North Lawndale community, providing clinical and psycho educational services to families from varying Chicago neighborhoods for the last 23 years. Opportunity: Clinical Case Manager for Mobile Crisis Program * Full time, employed position * Schedule: 24/5 program; flexible day or night shifts. 3 shifts per week with 1 4hr shift Temporary Schedule *Plans to upscale the mobile crisis program to 24/7/365 and schedule will be 40hr week with a flexible schedule with rotating weekends and nights. Must be able to accommodate a flexible full time schedule to meet the needs of the growing program. * Teams will consist of a Case Manager and a non clinical community outreach specialist. *Social Worker (MSW) and a Supervisor (either LCSW or LCPC) will be reported to after initial crisis intervention for post crisis stabilization and continuing therapeutic services. * Will service Cook County and Lawndale area Duties & Responsibilities: *Provide a continuum of care including working within a dyad by serving as the clinical lead to facilitate de-escalation, assessment and stabilization and to provide service care coordination and resource linkage to residents *Will work with a non clinical community outreach specialist to answer calls *Must be able to effectively communicate and assess patients experiencing a mental health crisis. *Must have strong interpersonal skills and be able to communicate effectively with a diverse patient population *Assist with crisis response, assessment and stabilization *Employ de-escalation techniques *Engage in collaborative engagement with residents to identify needs and develop a service plan *Complete the IM-Cat training and be HIPAA compliant Requirements: *Bachelor's Degree in Social Work, Counseling, Psychology and/or related field *2 years of case management experience in social services or healthcare setting *Experience working in inner city communities is highly preferred *Crisis Experience is a plus *Must have access to a vehicle, proof of insurance and a valid driver's license. Compensation: Annual salary of $50,000 with comprehensive benefits available.
