General Duties:
As a Licensed Clinical Social Worker (LCSW), you will play a crucial role in facilitating patient care along the continuum. Your responsibilities will include effective resource coordination to help patients achieve optimal health, secure access to necessary care, and ensure appropriate utilization of resources while respecting the patient's autonomy. Core responsibilities include:

Assessing patients for transition needs and identifying those at risk for readmission.
Conducting complex psycho-social assessments and interventions to promote timely throughput, safe discharge, and prevent avoidable readmissions.
Managing transitions to promote appropriate lengths of stay, reduce readmissions, and enhance patient satisfaction.
Coordinating care efficiently and ensuring that care is delivered at the appropriate level.
Ensuring compliance with state and federal regulations, TJC accreditation standards, and organizational policies.
Delivering education to physicians, patients, families, and caregivers.
Mentoring and precepting new staff members.
Participating in departmental quality improvement initiatives and major assigned projects.
Core Responsibilities Include:

Conducting psychosocial transition planning assessments and interventions.
Facilitating assessments related to adoption, abuse, and neglect, referring to necessary local or federal agencies as appropriate.
Leading multidisciplinary patient care conferences, including complex case reviews.
Mentoring new staff and acting as a resource for all team members.
Communicating effectively with patients and families regarding care plans.
Collaborating with physicians, office staff, and ancillary departments to ensure a seamless care experience.
Completing accurate patient demographic and insurance documentation in the Case Management system.
Qualifications:

Required: Current CA LCSW license, certified or registered to practice according to state regulations and policies.
Preferred: Accredited Case Manager (ACM) credential; a minimum of two years’ acute hospital experience.
Strong organizational, verbal, and written communication skills.
Proven ability to lead and coordinate diverse groups in a fast-paced setting.
Critical thinking and problem-solving skills, along with computer literacy.
Compensation:

Pay Range: 38.94−38.94−60.36 per hour
Base Salary: $82,000 to $126,000 annually
Sign-on Bonus: Up to $25,000
Individual compensation is determined based on qualifications and experience.
