Our County Mental Health Center is seeking a detail oriented Supportive Employment Case Manager within the Individual Placement Support team. Working with a case load of up to twenty clients, the Case Manager will provide the EBP employment program to adults with mental illness and/or substance use disorders. The Case Manager will also build strong relationships with community employers for customized job development.
Key responsibilities:
Assist clients to find employment opportunities that are consistent with their vocational goals by identifying individualized information regarding entitlements/benefits, strengths, and preferences.
Will meet with hiring managers and attend job fairs to learn about various jobs in the community.
Provide support to assist clients in maintaining employment once a position has been secured.
Provide education and negotiate job accommodations as needed.
Attend team meetings for case managers and QMHP’s.
Qualifications:
Bachelors degree in social sciences or related field or comparable work experience.
Must have valid driver’s license and ability to move freely around the community.
Must be able to meet productivity standards and documentation requirements.
